Work as part of the primary care team to support the emotional and behavioral health needs of patients. Provides Primary Care Physicians (PCP) consultation, assists in identifying “at risk” patients and developing a patient care plan. Provide brief solution-focused and motivational interviewing interventions to prevent further psychological or physical deterioration. Provides information and referrals to patients regarding resources for their basic needs behavioral health care, and other social services.
A master’s degree in Social Work with a license eligible to practice independently in North Carolina is required. At least two years of fully licensed practice experience in a primary care medical setting is strongly preferred.
